Return to Work Plan (RTW Plan) is a written action plan explaining how you will help your injured worker to return to work or how you will help them to remain at work whilst they recover.

You must prepare a RTW Plan within 10 days from the date your injured worker’s claim for weekly payments was accepted, or from the date you became aware your injured worker would have an incapacity for 20 days or more, whichever is the later.

If your injured worker has a current work capacity, the RTW Plan must, by law, include an Offer of Suitable Employment to assist them to return to work.

An Offer of Suitable Employment is a written outline of the suitable duties you have identified to return your injured worker to work and should be incorporated into the overall Return to Work Plan.

You can used this form, or create a version to meet your needs. If you create your own version, ensure all of the mandatory requirements are included.
